Available Positions

The trust is currently recruiting for the following part-time roles:

Role 1 – Assistant Practitioner (Domiciliary Therapies, Merton Localities)

  • Working Hours: 27 hours 45 minutes per week

Role 2 – Assistant Practitioner (Merton Enhanced Home-Based Therapy Team)

  • Working Hours: 23 hours 35 minutes per week

Key Responsibilities

Successful applicants will work alongside registered healthcare professionals to deliver high-quality rehabilitation and therapy services within the community. Main duties include:

  • Supporting the assessment of patients’ health and social care needs.
  • Assisting with the delivery and review of personalised care plans under the supervision of qualified healthcare professionals.
  • Working closely with Occupational Therapists to manage delegated patient caseloads safely and effectively.
  • Providing rehabilitation support to adults with physical and cognitive challenges in community settings.
  • Carrying out basic functional assessments to support ongoing patient care.
  • Collaborating with multidisciplinary teams, including GPs, social workers, hospital staff, and voluntary organisations.
  • Maintaining accurate clinical records and following NHS service guidelines.
  • Developing professional skills through continuous learning and training.
  • Recognising personal professional boundaries and escalating concerns whenever necessary.
