Available Positions
The trust is currently recruiting for the following part-time roles:
Role 1 – Assistant Practitioner (Domiciliary Therapies, Merton Localities)
- Working Hours: 27 hours 45 minutes per week
Role 2 – Assistant Practitioner (Merton Enhanced Home-Based Therapy Team)
- Working Hours: 23 hours 35 minutes per week
Key Responsibilities
Successful applicants will work alongside registered healthcare professionals to deliver high-quality rehabilitation and therapy services within the community. Main duties include:
- Supporting the assessment of patients’ health and social care needs.
- Assisting with the delivery and review of personalised care plans under the supervision of qualified healthcare professionals.
- Working closely with Occupational Therapists to manage delegated patient caseloads safely and effectively.
- Providing rehabilitation support to adults with physical and cognitive challenges in community settings.
- Carrying out basic functional assessments to support ongoing patient care.
- Collaborating with multidisciplinary teams, including GPs, social workers, hospital staff, and voluntary organisations.
- Maintaining accurate clinical records and following NHS service guidelines.
- Developing professional skills through continuous learning and training.
- Recognising personal professional boundaries and escalating concerns whenever necessary.
